DHC Express aims to provide primary healthcare services including health promotion, assessment and patient empowerment program to enhance the public awareness on disease prevention and the capacity in the self-management of health through the public-private partnership. Key Responsibilities Coordinate, plan and deliver health assessment and complication screening service, patient empowerment programs and individual health coaching Set up the systems for documentation, communication and service monitoring Network with different medical and community partners to develop and implement the primary health promotion services Provide staff supervision and coach the health promotors Offer Health Assessment/ Screening/ education Responsible for dedicated nurse clinic Requirements Degree holder or above in nursing discipline, Registered Nurse Complete Post-registration Certificate Course in Community Nursing/ Primary Health Care/ Public Health are preferable Five aggregate years or above work experience in patient nursing care/ Primary Healthcare, especially in patient education and community health nursing Strong leadership with good decision making and analytic skills Able to work independently with good problem-solving skills and flexibility Excellent communication and interpersonal skills Proficiency in use of IT Technology Competency in stakeholder engagement Good command of written and spoken English, Cantonese and Mandarin Shift duty including 1-2 days night shift until 9:00 p.m. per week is required The successful candidate will normally be appointed on contract terms. Renewal of contract will be subject to the service/operational needs of the department and the candidate’s performance.
