Job Overview
As a Physician Case Manager at Teladoc Health Asia, you will play a vital role in remotely coordinating and managing the medical care of patients. About the Role
This is a part-time consulting opportunity for a mid-senior level professional with a medical degree from an accredited university/institution. You will serve as the primary point of contact for patients, healthcare providers, and other involved parties, ensuring effective communication and coordination.

Responsibilities
  • Manage and follow up cases referred by Teladoc Health Asia.
  • Serve as the liaison between patients, healthcare providers, and other stakeholders, ensuring effective communication, care coordination, and optimal health outcomes.
  • Conduct comprehensive assessments of patients' medical conditions, symptoms, and treatment history.
  • Collaborate with patients to develop personalized care plans, considering their unique needs, preferences, and available resources.
  • Continuously monitor and evaluate patients' progress, adjusting care plans as necessary.
