Qualifications A University Degree in Social Work and registered under Social Workers Registration Ordinance. English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE/Level 2 in HKDSE. 2 years of post‑qualification experience in community health care as a Social Worker preferred. Experience working in cancer patient support preferred. Strong ability to establish and maintain positive and effective work relationships with volunteers, clients and partners. Good interpersonal and communication skills; dynamic, independent, collaborative. Job Duties To plan, implement and monitor Cancer Survivorship Project including case management, training sessions/workshop, outreach activities for symptom management. Assess the needs of all registered cases and formulate cases' health intervention. Provide community resources/interventions to strengthen the knowledge and skills in cancer support in the community. Deliver health education to the public to promote health literacy. Develop training materials and guidelines. Co‑work with clients and academics in progress and outcome documentation and evaluation.
