Social Worker

Responsibilities Improve the social determinants of health, including support for the engagement of the DHC members and their families; implementation of medical‑social collaboration and community engagement under the DHC Scheme Develop and provide community rehabilitation, caring and support plan and psycho‑social support services, counselling services for people with chronic diseases and caregivers Organize health education activities, support Self‑management Support Program (SMSP), Patient Empowerment Program (PEP) and other community support programs Provide social work and psycho‑social intervention and counselling services Liaise with local community stakeholders, and develop partnership in the community Develop volunteers training and development programs Requirements Registration with the Social Workers Registration Board Five aggregate years of practice in social or medical service upon assumption of duty Self‑initiative, independent, mature, flexible and good interpersonal skills 45 working hours per week, 5 days’ work, shift duties are required, may need to work in the evening, Saturdays, Sunday & Public Holidays subject to operation needs Interested parties please click “Apply" to submit your full resume withemploymenthistory, expected salary.
