Care Coordinator / Registered Nurse (Ref: DHC-CC/RN-11/2025-JDB) Responsibilities Coordinate and provide nursing care to Southern District Health Centre clients within the community, including health assessment, screening, health coaching, and the planning and implementation of chronic disease management and community rehabilitation programmes. Develop clinical guidelines and workflows for nursing services. Collaborate with the Southern District Health Centre multi‑disciplinary team to develop and conduct health promotion and patient education programmes, as well as related training. Liaise with stakeholders such as government officials, network medical practitioners, professional groups, district stakeholders, and other relevant parties. Qualities & Requirements Holder of a valid Part I or Part II Registered Nurse practising certificate issued by the Nursing Council of Hong Kong. Degree in Nursing or equivalent. Minimum 5 years of aggregate experience as a registered nurse, including supervisory experience in patient‑care nursing practice, preferably within public services or NGOs. Experience in vaccination, phlebotomy and intravenous infusion is preferred. Recognised post‑registration training in Primary Health Care, Community Care, Diabetes Mellitus care, Cardiology, Stroke Rehabilitation, Cardiac Rehabilitation or Gerontology is an advantage. Good communication and interpersonal skills. Excellent spoken and written proficiency in English and Chinese. Shift duty including 2 days of night shift until 9:15 pm each week. #J-18808-Ljbffr
